INSURANCE
Whole Life OT is a credentialed, in-network provider with Medicare Part B.
For clients with Medicare Advantage plans or private insurance, I am able to provide services as an out-of-network provider.* Self-pay rates are listed below. Upon request, I can provide the necessary documentation for you to submit to your insurance provider for reimbursement (called a “superbill”).

RATES
Evaluation/Re-evaluation (60-75 minutes): $145-175
60-minute Treatment Session: $128
45-minute Treatment Session: $96
Note: These rates reflect a 20% prompt pay discount for clients who are paying out of pocket at time of service.
GOOD FAITH ESTIMATE
In accordance with the No Surprises Act, Whole Life OT, PLLC provides a Good Faith Estimate to self-pay clients and clients using out-of-network insurance benefits. Learn more about the No Surprises Act at https://www.dol.gov/agencies/ebsa/laws-and-regulations/laws/no-surprises-act.
PAYMENT
All payment is due by the day of service through the patient portal. For Medicare participants: Co-pays, co-insurances, and deductibles are due at the time services are rendered.
